ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNIFICATION AGREEMENT
By signing this waiver, I acknowledge and agree to the following:
- I understand that participation in aqua park activities involves inherent risks, including but not limited to: risk of serious injury, death, property loss or damage, and other potential dangers.
- I voluntarily assume all risks associated with participation, whether those risks are known or unknown.
- I hereby release, waive, discharge, and covenant not to sue the aqua park, its owners, operators, employees, agents, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained while participating in activities or while on the premises.
- I agree to indemnify and hold harmless the aqua park from any loss, liability, damage, or costs that may incur due to my participation.
- I understand that this waiver is binding upon my heirs, next of kin, executors, and administrators.
- If signing as a parent or legal guardian for a minor, I have full authority to bind the minor to this agreement and I agree to indemnify the aqua park for any claims brought by or on behalf of the minor.
- I certify that I am in good physical condition and have no medical conditions that would prevent safe participation.
- I have read this waiver carefully and understand its contents. I am aware that this is a release of liability and a contract between myself and the aqua park.
